The consent forms are available for download here. You do not need to print out the document yourself. The researchers will have consent forms with them on each observation day, which you can sign on site.

Are you a member of the medical staff in one of the emergency rooms?

Are you being/have you been treated in one of the emergency rooms or have you accompanied a person to the emergency room who was treated there?
